My superjay day of being 1st AM active.
1) received call from ot baby nurse, to come for standby for ellscs for mother sedated with GA
- attended
- baby born flat 😶
- pinjam steth dr anesth
- HR < 100❗i gave ppv
- baby not respond
- pakar anesth lompat mai dekat warmer
- "baby flat" dia jerit (call for help)
- dia take over ppv
- mr sopa
- oxygen tube tercabut dari humidifier rupanya 😭
- baby subsequently crying pink vigorous
Ma shaa Allah yg memegang nyawa manusia (tadi flat kalo biar boleh ko 😭)
- but then baby grunting
- accompany baby in incubator, admitted with cpap to nicu
Returned to do ward works. Took some blood and tackle branula. Ma shaa Allah mudahh. Hari lain jarang la yg sekali poke dapat. Sorang2 plak tu secure. Alhamdulillah Allah tolong.
Then dapat call lagi.
2) received a call, that got an ellscs (again) baby bornt but noted subcostal recession.

- lungs equal air entry
- mild to moderate scr
- not tachypneic 😔
- no nasal flaring
- pink
- i put the baby under nasal prong 2l/min
- the condition remains same, not worsening
- i think i might want to consider cpap later, will ask mo
- i look at the phone
- 'farhana, fetal distress at another ot now!' my mo tell in the group
- i feel suffocated
- o&g team hasnt yet call me
- my 2nd active is a junior, i call her informing the fetal distress
- she didnt know where's another ot located
- shit
- i look at the baby i'm attending, still having scr under npo2, this baby need further assessment
- i rush to attend fetal distress, leaving my current baby to my 2nd active
- this is the most difficult and not so good decision i've made.
😔 Judging that the baby not tachypneic, doesnt mean the baby okay.
3) attended the emlscs for fetal distress at another ot.
- baby born vigorous, pink, apgar 10.
- alhamdulillah.
- despite bad ctg.
- subhanAllah. Plot twist.
Just about to go out of ot a,
Received another two referals.
4) was told baby has facial congestion at 1 hour of life.
- attended to labor room
- baby was very comfortable
- very minimal / mild facial congestion
- discussed with mo, baby is discharged but must take fbc at pnw
- done a very fast assessment because have to attend another awaiting referal
5) attended for standby for ellscs for refused tolac but underlying maternal gestational thrombocytopenia, sedated for GA
- i arrived at the ot before the op started
- got some time to reflect
- was told that the previous baby that i attended and left to my 2nd active for further assessment, was admitted for cpap. Poor baby. I'm sorry i can't help you further earlier. I am sorry 😞
- back to reality
- baby also born flat, maternal sedated, but this time my mo is around
- baby cry out loud after initial resus
- alhamdulillah
- settled the clerking, then went to inform the o&g surgeon
- he's doing closure
- dr t, i'm f ho paeds, i want to inform baby is discharged to mother.
- oh okay. Dont u guys worry about baby having thrombocytopenia?
- we would like to repeat Full blood count in post natal ward doctor. Also we will admit if mother is admitted to icu.
- he nodded. 'no, the mother wont be admitted to icu. Thank you ya'

Then i pergi sembahyang zuhur.
Balik nicu utk simpan barang2 resus yg dibawa sejak 4 cases yg lalu. Niat nak pi makan,
Then balik ward dr ward panggil buat kerja.
Alhamdulillah.
Kena bersyukur.
Kerja ward bertimbun sebab yg kena tinggal jaga is also 1st poster. Knowing his previous performance, i consider he has improved a lot. At least he managed to take all the blood since i left for active few hours (4 cases back to back) ago.
I do referal to psy for infant of mother having psy issue (at 415pm haha), tracing of morning blood result, then came few parents:
"dr discharge anak saya bila nak siap?"
Few same questions.
And some asking about the conditions of their baby.
"how is my baby?"
And staff nurses complaining about discharges havent been done yet.
Yes complain. Not helping at all.
Even tho i feel annoyed, that i just came back from 4 active cases, at 3 different locations with having to change to scrub multiple times, carrying along resus box and portable cpap,
I just decided to entertain those patients. And those nurses.
Amazing. When i replied to each of them politely, they spoke more polite than me.
I answer their questions and worries. I reply them. I attended them.
Eventho my stomach churning for food.
Because i know, waiting for answer/query / just waiting for anything, is what makes people less patient and triggered.
Then when the visiting time ended, i managed to sit down properly and do the discharges. Nobody disturbed me nor i have to stand up few times attending their call/worries.
Went to solat asar then continue my worldly works.
SubhanAllah. Yesterday was my best day. Because Allah help me a lot. Everything was beyond my capability. Everything is of His help and guidance.

P/s. At previous night shift, i also got 5-6 referals. 😅 But yesterday was my first ppv. Hahahaha
